Hip pain at night usually comes from one of two things: pressure, when a hip presses into a surface that is too firm, or alignment, when a surface that is too soft lets the hip drop and twists the pelvis. The fixes follow from that: sleep on your side with a pillow between the knees or on your back with one under them, cushion the hip with a pressure-relieving mattress, stretch the muscles around the joint in the evening, and see a professional if the pain is persistent. Here is how to put each into practice.

Key takeaways

  • Side sleepers: a pillow between the knees keeps the pelvis level and takes load off the top hip.
  • Back sleepers: a pillow under the knees relaxes the hip flexors and lower back.
  • A medium or medium-plush hybrid with memory foam or latex relieves hip pressure while supporting the spine.
  • Gentle evening stretches for the hip flexors, glutes and piriformis help; hard workouts before bed do not.
  • Chronic or severe hip pain needs a diagnosis, not just a new bed.

Whether it comes from an injury, arthritis, bursitis or simply years of sleeping on the wrong mattress, hip pain makes it hard to find a position you can stay in. The good news is that a few changes to how and what you sleep on make a real difference, often within a week.

Why Hips Hurt at Night

Lying down for eight hours loads the hip joint in a way standing never does. On your side, the whole weight of your torso rests on one hip; if the mattress is too firm, the joint and the bursa over it are compressed all night. If the mattress is too soft or sagging, the hip sinks, the pelvis tilts and the muscles and ligaments around the joint are stretched. Arthritis and bursitis make both worse because the joint is already inflamed. Our guide to the side-sleeper shoulder pinch explains the same mechanics one joint higher.

Test Different Sleeping Positions

  • On your side with a pillow between the knees. This keeps the top leg from pulling the pelvis forward and reduces pressure on both hips. Sleep on the less painful side where possible.
  • On your back with a pillow under the knees. Takes tension off the hip flexors and flattens the lower back; the most joint-friendly position for most people with hip pain.
  • Avoid stomach sleeping, which rotates the hips and arches the lower back.

Finding the right position takes some trial and error, but it is the cheapest fix and often the most effective.

Incorporate Gentle Stretching

Tight muscles around the hip, the hip flexors, glutes and piriformis, pull on the joint and make it ache at rest. Ten minutes of gentle stretching in the evening helps: a kneeling hip-flexor stretch, a figure-four stretch for the piriformis, and a knee-to-chest stretch for the glutes and lower back, each held for 30 seconds. Keep it gentle and slow. Intense exercise within two hours of bed raises your core temperature and heart rate and makes falling asleep harder.

Use Supportive Pillows

A contoured pillow that keeps the neck in line with the spine matters more than it seems, because a twisted neck changes how you lie all the way down to the hips. A firm knee pillow or a standard pillow between the knees for side sleeping, or under the knees for back sleeping, is the single most useful accessory for hip pain. See our pillows and accessories.

Invest in the Right Mattress

The mattress has to do two things at once: cushion the hip so it is not pressed against a hard surface, and support the pelvis so it does not sink. That is the definition of a good hybrid: memory foam or latex comfort layers over pocket coils. For most people with hip pain a medium to medium-plush feel is the sweet spot; heavier sleepers should go medium-firm so the hip does not bottom out. A sagging mattress with a dip under the hips will undo every other fix, so if your bed is more than 7 or 8 years old, start there.

Consult a Healthcare Professional

If hip pain is chronic, severe, wakes you at night, or comes with swelling, clicking or reduced range of motion, see a doctor or physiotherapist. Conditions such as osteoarthritis, bursitis, labral tears or referred pain from the lower back need a diagnosis and a treatment plan, which may include physiotherapy, medication or in some cases surgery. A better mattress supports recovery; it does not replace it.

Frequently asked questions

What is the best sleeping position for hip pain?

On your back with a pillow under the knees, or on your non-painful side with a pillow between the knees. Avoid sleeping on your stomach.

Is a firm or soft mattress better for hip pain?

Neither extreme. A medium to medium-plush mattress with a pressure-relieving top over a supportive core works best; too firm compresses the hip, too soft lets it sink and tilts the pelvis.

Can a mattress topper help hip pain?

If your mattress is supportive but too firm, a 2 to 3 inch memory foam or latex topper can cushion the hip effectively. If the mattress sags, a topper will not help.

Why does my hip hurt only at night?

Lying still loads the joint for hours and inflamed tissue stiffens without movement. Pressure from a firm surface or poor alignment on a soft one makes it worse; morning stiffness that eases with movement often points to the bed.
